In order for us to get started on your services, we will need THe following:

1) Your family history

This will provide us with all of the information we need to start ordering your documents: 

  • For jure sanguinis applicants (via ancestry): (click here)

  • For jure matrimonii applicants (via marriage): (click here)

Please note:

  • This step is not necessary if you are procuring your own U.S. and Italian Vital Records.

  • You may leave any sections blank that do not apply to the records we are procuring.

  • If you are applying with multiple family members, one of you will need to fill out their personal information along with the information for the common line of ancestry. The other/s can proceed only with the information that does not overlap.


2) SIGNED PERMISSION FOR US TO PROCURE CERTIFICATES ON YOUR BEHALF

A) FOR U.S. VITAL RECORDS:

We will need a notarized power of attorney (click here) along with a driver's license or legal identification card. 

We are required to submit this along with the request forms for all U.S. vital records. If possible, please try to have three copies notarized as certain agencies will not return the hard copies. Once you have these, please send a digital copy to us by email and the paper copies to the address reflected on your invoice.

Please note:

  • If you are applying with multiple family members, each of you will need to have a power of attorney notarized (except for minors).

  • If you have documents from New Jersey, Connecticut or New York there will be additional steps to take in order to procure your documents. One of our members of staff will be in touch with you with further information on this.

B) FOR ITALIAN VITAL RECORDS:

We will need a signed (not notarized) form (click here) along with a driver's license or legal identification card. 

We are required to submit this along with the request forms for Italian vital records. Once you have this, please send a digital copy to us by email, paper copies are not necessary. If you do have scans of the records or any other supporting information, please share this with us by email.


3) A down payment of half of the fee reflected on your invoice

The other half is payable upon completion of our services. We accept check (no service fee) or credit card.

We accept most major credit cards. You may send the down payment via a link on your invoice (does include a service charge of 2.9% + $0.30 for most credit cards and 3.25% + $0.30 for American Express).

This step of the process is very important and should be started as early as possible so that you may collect your documents while you are waiting for your appointment. It requires a lot of patience due to the high demand and extremely limited supply - they often fill up quickly so you will need to be persistent in checking back often, and also patient in understanding that sometimes they book years in advance. Do not let this discourage you!
